APPLICATION PROCEDURE

Applicants should complete an on-line application form and indicate;

1. their previous clinical and research activities in the field of diabetes/epidemiology or general experience in diabetes education
2. their expectations from the course
3. their plans about application of the knowledge they will acquire from the course on return to their home countries

Curriculum vitae should be included.
Applicants should be supported and recommended by their institution.

Completed application form with signature and a letter of recommendation
should be;

Applicants will receive notification by e-mail or fax of the receipt of the application form within 3 days of receipt.

Notification of admission to the course will be e-mailed or faxed to those who are accepted by June 30, 2008.

An official letter of invitation will be issued upon request.
